Just read about a massive recall involving 1.3M bikes produced since 1998 from 17 different manufacturers. Basically it sounds like if your bike shipped with disc brakes and a QR front axle, it’s part of this recall. Here’s the official description:
The recall involves bicycles equipped with front disc brakes and a black or silver quick-release (QR) lever on the front wheel hub. When the front QR is fully opened, if there is less than 6 mm — or the width of a #2 pencil — between the QR and disk brake rotor on the wheel, the bicycle is included in this recall.
Affected brands include: Diamondback, Raleigh, Breezer, Fuji, Felt, Jamis, Giant, Haro, Norco, Performance Bike, REI, and Specialized. It sounds like each company is handling the replacement process on their own so contact your manufacturer to get details. In most cases the fix involves taking your bike to a local shop to get a new QR swapped in.
